This cylindrical box was in the past used by poor families to deliver anonymously babies to be raised and fed by the nuns - something that the babies' parents hardly could do.
A baby was put in that box, on the outside wall of Arouca Monastery. A rotation of the box would show its content to anyone passing by the inside wall.
