put the onions and the garlic together with two spoons of sambal oelek into an ulekan (pestle and morter). If you want the dish to be hot, add three spoons, and if you want the dish to be less hot, only add one spoon.

Let simmer for at least an hour. Best to let it simmer for a couple of hours. Stir occasionally. If it gets to dry, add a bit of water. The sauce must thicken, so don't overdo the water and keep the heat low. Serve with rice. Remove the sereh before serving.
