I drank macchiatos in Sicily last month. They told me there was no real coffee in it, but it tasted the same. Since, I've found out that there is some espresso in it, which explains the taste - I used to love espresso until I became caffeine intolerant. Having had no ill effects drinking it in Sicily, I'll have an occasional treat of it around here now - if I can find anyone who will make the drink.

Point is: the more water passes on the grinded coffee more it releases caffeine. That is why Italians and Portuguese drink small expressos called "ristretto" because flavour is what comes out first, and a ristretto has almost no caffeine, that is why you felt better...
Tea is the opposite: you pour water, leave it for 10 seconds and drop it, add new water and you will have a detheinated tea... in tea theine comes out before flavor.
