On 22/1/2010, 158701 derailed on the approach to Dingwall while working 2H75, the 17:15
Inverness - Ardgay. There were no serious injuries and passengers and crew evacuated the train by ladder.
The RAIB have since publised THIS report, from which you can draw your own conclusions.

If you're going to derail a 158, you might as well put all the wheels in the ballast and do it at the throat of a junction station. Joking aside though, and the most important thing, there were no serious injuries.
