Near St. George, Utah, USA. The southeast dike of the dam collapsed in the wee hours on New Year's Day, 1989, after being undermined due to gypsum in the country rock dissolving out. There were no injuries but a considerable amount of property damage. The dike has since been rebuilt with a much better foundation.
